These are fluid attenuated inversion recovery (FLAIR) magnetic resonance imaging (MRI), gadolinium enhanced T1-weighted MRI, and 11C-[R]-PK11195 positron emission tomography images (on the top, middle and bottom fig, respectively) in a 5.5-year-old boy with X-linked adrenoleukodystrophy. 11C-[R]-PK11195 positron emission tomography (PET) is showing increased tracer binding in (a) occipital, parietal and posterior temporal white matter and (b) posterior most part of the posterior limb of internal capsule suggesting neuroinflammation mediated by activated microglia. Increased tracer binding is also seen in bilateral posterior thalami (c) probably due to primary involvement or trans-axonal activation of microglia in the connected thalamic nuclei.
